Summer Enrollment Forms

You will need to print off the 2 forms below and turn them into the studio to enroll. Enrollment begins Monday, April 16th at 4:30pm.  You can also e-mail Carrie at powerhousedance@comcast.net.  If you do e-mail in your enrollment then you will need to mail your forms along with payment to 1750 East Harold Street or bring it into the studio that week.  If we don't get your forms or payment within a week we will have to take your dancers name off of the class roster.  Make sure that you have both forms printed off to enroll.  We are looking forward to getting to teach your dancer this summer!
